At A Dream Called Hope, we believe that successful treatment extends beyond a youth's time in residential care. From the day a youth is admitted, our team works collaboratively with families, guardians, service providers, schools, and community partners to develop a plan that supports lasting stability, continued growth, and successful reintegration into the community.

Through ongoing communication and coordination, we strive to ensure that services remain connected, goals remain aligned, and youth have the support necessary to succeed during and after treatment.

Care Coordination Services May Include:

  • Collaboration with guardians, families, and caregivers

  • Coordination with case managers and care coordinators

  • Communication with behavioral health providers and therapists

  • Collaboration with psychiatrists, medical providers, and treatment teams

  • Coordination with schools, teachers, and educational personnel

  • Participation in Child and Family Team (CFT) meetings

  • Collaboration with probation officers and juvenile justice professionals when applicable

  • Communication with DCS specialists and placement teams when applicable

  • Participation in treatment planning and service coordination meetings

  • Progress updates and treatment recommendations as authorized

  • Advocacy to ensure services align with the youth's individualized needs and goals

  • Coordination of community resources and support services

Discharge Planning Begins at Admission

We believe discharge planning is most effective when it begins early. Throughout a youth's stay, our team works to identify strengths, address barriers, and prepare for a successful transition to the least restrictive and most appropriate environment.

Discharge planning may include:

  • Identification of discharge goals and expectations

  • Ongoing assessment of progress and readiness

  • Family and guardian involvement in transition planning

  • Coordination of outpatient behavioral health services

  • Coordination of psychiatric and medical follow-up care

  • Educational transition planning

  • Development of safety and crisis prevention plans

  • Independent living skill development

  • Community resource identification and referrals

  • Housing and placement coordination when applicable

  • Connection to mentoring, recreational, and supportive services

Aftercare & Continued Support

A successful discharge is not simply leaving a program—it is having the resources, relationships, and support necessary to continue progressing after treatment.

Our team works to establish ongoing supports that may include:

  • Referral and linkage to outpatient therapy services

  • Behavioral health provider coordination

  • Medication management follow-up recommendations

  • School transition support

  • Community-based support services

  • Family support and resource referrals

  • Vocational and educational resources when appropriate

  • Coordination with case managers and community partners

  • Ongoing communication during transition periods as appropriate

Maintaining Healthy Connections

At A Dream Called Hope, we understand the importance of positive relationships in the healing process. When clinically appropriate and operationally feasible, former residents may have opportunities to maintain healthy connections with supportive staff, peers, program activities, and other positive influences developed during treatment.

We believe that continued connection and support can play an important role in reinforcing progress, promoting resilience, and encouraging long-term success following discharge.
